Kingsbrook celebrated the opening of its new outpatient center with the
help of community leaders, elected officials, managed care affiliates
and staff. The center which has been in the works for almost 2 years ago
was designed to support Kingsbrook's ongoing commitment to improving
overall quality of care and to transform how care is delivered in response
to the unique patient population; keeping in mind the changes health care
reform legislation is bringing. Kingsbrook's new center provides truly
integrated care, one stop shopping, with a team of physicians representing
several specialties all practicing together in one location and using
a common computerized health record. At this new center care is individualized
and coordinated--a unique way of practicing quality medicine in central Brooklyn.
Additionally, Kingsbrook is the very recent recipient of a $1,000,000 grant
from the Centers for Medicare and Medicaid (CMS). This grant is integral
to how this new facility will be used and will allow Kingsbrook to implement
a new model of care for patients, a model known as patient-centered medical
home, PCMH, a care delivery model that places the patient and family literally
at the center of how their care is provided. This model has the potential
to dramatically improve the health of those suffering from multiple medical
conditions such as diabetes, heart failure, or chronic lung ailments.
