SPECIAL NOTICE/ FICA REFUNDS
Persons who were Medical or Dental Residents, or Clinical Fellows, at Kingsbrook Jewish Medical Center at any time during the period January 1, 2000- December 31,2003, and who were paid by the Hospital during that time period, may qualify for IRS refunds of FICA tax amounts that were deducted from their pay. These refunds apply only to qualifying persons who meet these criteria. The IRS explains the FICA tax refunds on its Website at (http://www.irs.gov/newsroom/article/0,,id=219731,00.html).
Kingsbrook Jewish Medical Center has engaged Healthcare Affiliates, Inc., (HAI), a financial consulting firm, to process the claim to the IRS, on behalf of the Hospital and all of the qualifying former residents that will result in the payment of the FICA tax refunds, including interest. The IRS has developed an "Employee Consent Form" that individual former residents must complete & sign, for the purpose of authorizing (name of hospital) to file the claim on their behalf.
In order to qualify for a FICA tax refund, an applicable former resident must complete the Employee Consent Form and deliver it to HAI September 23, 2010. HAI will accept the completed forms in one of three ways: 1) by mail to Healthcare Affiliates, Inc., 1 Vale Rd., Suite 200, Bel Air, MD; or 2) by fax to 410-879-3266; or 3) scanned in an acceptable format and attached to an email addressed to kjmc@healthcareaffiliates.com. HAI will confirm receipt of properly completed Employee Consent Forms upon request.
In conversations with the IRS, they have stated emphatically that they want to see a significant effort on the part of hospitals in contacting former residents; in fact, they have suggested that they "expect" an 85% rate in order for the Hospital's claim to be deemed acceptable. Therefore, we are requesting that each former resident spread the word to former classmates to contact HAI as soon as possible.
